The Hoffman Award Application Process:

The following must be received no later than Friday, January 22, 2010:

1. Application Form - Click here to view and print the form.
   
   
2. One-Page Essay - Describe how your volunteer efforts have been extraordinary. Your essay must be typewritten.
   
   
3. Two Letters of Endorsement - These are to be written by the leader/supervisor of the organization(s) for which you have performed community service duties and/or volunteer activities. Your letters must be submitted on official letterhead.
